Abstract:

We present a case report of a 63-year-old male who underwent lung resections for metastases originating from gastric cancer 18-year after total gastrectomy with lymphadenectomy. The gastrectomy was performed in 1994; histological examination of the original tumor revealed stage II poorly differentiated adenocarcinoma [pT2 (MP), N0, M0]. Chest X-ray and computed tomography in 2012 showed a well-defined tumor, 9 mm in size, at the left S3 of the lung. Thoracoscopic partial resection was performed. The tumor was diagnosed as poorly differentiated carcinoma, most likely metastatic gastric adenocarcinoma. Although rarely performed, resection of pulmonary metastases from carcinoma of the stomach was done to improve the patient’s chances for long-term survival.

Abstract:

We present a case report of a 65-year-old woman who underwent resection of a chest wall tumor. In contrast with computed tomography, the tumor exhibited hypervascularity and was fed from the 9th intercostals artery. Histologically, the tumor was diagnosed as a cellular variant of solitary fibrous tumor of the pleura, with branching “staghorn” vessels and hypervascularity. The tumor thus resembled a chest wall hemangiopericytoma.

Abstract: We have experienced three inverted Meckel’s diverticula with ileus that are diagnosed before surgery and treated with laparoscopic surgery. The patients were 22, 26, and 29 years of age. The history of ileus for each patient differed, being the first experience for one patient, the second for another, and the third experience for the remaining patient. None of these patients had a history of abdominal surgery. Conservative therapy eventually improved the existing symptoms, but the cause of past ileus remained unclear. Elective surgery was performed 12 days from the onset of symptoms on two of the patients after improving the ileus by inserting a long tube into the bowel. Emergency surgery was performed on the same day of admission on the remaining case that involved moderate expansion of intestine. In these patients, the first trocar was inserted into the umbilicus region. Laparoscopic surgery was then performed with three trocars. The patients were discharged upon favorable outcome after surgery. No recurrence of ileus was noted post-surgery. Laparoscopy was useful both to diagnose and treat ileus of a Meckel’s diverticulum.
DNS Analysis on the Indirect Relationship between the Local Burning Velocity and the Flame Displacement Speed of Turbulent Premixed Flames  [PDF]

Abstract: The local burning velocity and the flame displacement speed are the dominant properties in the mechanism of turbulent premixed combustion. The flame displacement speed and the local burning velocity have been investigated separately, because the flame displacement speed can be used for the discussion of flame-turbulence interactions and the local burning velocity can be used for the discussion of the inner structure of turbulent premixed flames. In this study, to establish the basis for the discussion on the effects of turbulence on the inner structure of turbulent premixed flames, the indirect relationship between the flame displacement speed and the local burning velocity was investigated by the flame stretch, the flame curvature, and the tangential strain rate using DNS database with different density ratios. It was found that for the local tangential strain rate and the local flame curvature, the local burning velocity and the flame displacement speed had the opposite correlations in each density ratio case. Therefore, it is considered that the local burning velocity and the flame displacement speed have a negative correlation.
Discrepancy between Patients’ and Nurses’ Estimates of Patients’ Activities of Daily Living for Fall Risk Assessment: A Quantitative Observational Study  [PDF]

Abstract: Background: Patient falls are a serious problem in a rehabilitation unit. Although patient falls have been described in the healthcare literature for more 60 years, and many risk assessment tools have been developed, the rate of falls in hospitals in Japan has remained unchanged for the last 8 years. A previous study reported that about 50% of patients in rehabilitation estimated their fall risk lower than that estimated by their nurses. We believe that patients in rehabilitation tend to overestimate their ability to perform ADLs. Aim: To identify discrepancies between patients’ and nurses’ estimates of patients’ ability to perform activities of daily living (ADL) and clarify any relationship between the discrepancies and patient falls. Methods: Participants comprised 82 patients (42 men) admitted to a rehabilitation unit in Osaka, Japan from July to December of 2017. Patients and their nurses answered the same questionnaire about patients’ ability to perform ADL. The questionnaire was developed based on the Functional Independence Measure (FIM) and administered at admission, at 1 month after admission, and at discharge. Participants were classified into the overestimating group and the accurately estimating/underestimating group, and groups were compared using Wilcoxon rank-sum tests. Results: The mean age of participants was 76.4 years. At admission, approximately 72% of participants estimated their own ability to perform ADL higher than did the nurses. The percentage of overestimating participants dropped to 30% at discharge. Fifteen of the participants experienced a fall; all were in the overestimating group. The ADL Discrepancy and fall-assessment scores for these 15 participants were significantly higher than those of other participants. Conclusions: There are discrepancies between patients’ and nurses’ estimates of patients’ ability to perform ADL and had important significance for assessing their risk of fall. And minimizing the discrepancy may support the prevention of falls.
Development of numerically controlled local wet etching

Abstract: Numerically controlled local wet etching (NC-LWE) has been developed as a novel noncontact subaperture deterministic figuring method for fabricating ultraprecision optics or for finishing functional materials. In this method, a localized wet etching area is formed using a combined nozzle that is constructed by coaxially arranging the supply part and the suction part of the etchant. The removal volume anywhere on the workpiece surface is determined by the convolution of the removal function and the dwelling time distribution of the nozzle. The machining properties of this method are insensitive to external disturbances, such as vibration or thermal deformation, because the removal process is performed under a noncontact condition.
Extraction of an Entanglement by Repetition of the Resonant Transmission of an Ancilla Qubit

Abstract: A scheme for the extraction of entanglement in two noninteracting qubits (spins) is proposed. The idea is to make use of resonant transmission of ancilla qubit through the two fixed qubits, controlled by the entanglement in the scatterers. Repetition of the resonant transmission extracts the singlet state in the target qubits from their arbitrary given state. Neither the preparation nor the post-selection of the ancilla spin is required, in contrast to the previously proposed schemes.
Non-Gaussianity of quantum fields during inflation

Abstract: In this review, we discuss how non-Gaussianity of cosmological perturbations arises from inflation. After introducing the in-in formalism to calculate the $n$-point correlation function of quantum fields, we present the computation of the bispectrum of the curvature perturbation generated in general single field inflation models. The shapes of the bispectrum are compared with the local-type non-Gaussianity that arises from non-linear dynamics on super-horizon scales.
Quantum Coherence of Electrons Field-Emitted from a Superconductor: Correlations and Entanglement

Abstract: The correlations of the electrons field-emitted from a superconductor are fully analyzed, both in space and time. It is proposed that a coincidence experiment would reveal a positive correlation between the electrons emitted in opposite directions. The electrons can be entangled and can even violate Bell's inequality. The crucial role played by Andreev's process is scrutinized, analytical formulas are derived for the correlations, and the physics behind the phenomenon is clarified.
Radion and Large Scale Anisotropy on the Brane

Abstract: We investigate the effect of the radion on cosmological perturbations in the brane world. The S^1/Z_2 compactified 5D Anti-de Sitter spacetime bounded by positive and negative tension branes is considered. The radion is the relative displacement of the branes in this model. We find two different kinds of the radion at the linear perturbation order for a cosmological brane. One describes a "fluctuation" of the brane which does not couple to matter on the brane. The other describes a "bend" of the brane which couples to the matter. The bend determines the curvature perturbation on the brane. At large scales, the radion interacts with anisotopic perturbations in the bulk. By solving the bulk anisotropic perturbations, large scale metric perturbations and anisotropies of the Cosmic Microwave Background (CMB) on the positive tension brane are calculated. We find an interesting fact that the radion contributes to the CMB anisotropies. The observational consequences of these effects are discussed.
